REQUEST FOR UNIFORM MITIGATION INSPECTION
VERIFICATION FORMS (FORM 1804) |

Due to the expiration
of the My Safe Florida Home program on June
30, 2009, the program does not have
the ability to provide or modify Form 1804
or 1802 - Uniform Mitigation Inspection
Verification Form.
Homeowners who did not
receive a mitigation form will need to
contact the inspection company that
performed the original inspection or have a
new inspection completed.

companies who performed
inspections statewide for the My Safe
Florida Home program. If you did not
receive a mitigation form following the
inspection of your home please contact the
appropriate inspection company to obtain a copy.
